A PILOT scheme to improve the care of people living with long- termchronic illness has been launched in Wales with Cardiff selected as one of the three areas to develop better services.
The aim of the plan announced by Edwina Hart, Welsh Health Minister, is to ensure that patients with arthritis, diabetes and heart and lung conditions have access to and receive health services quicker and nearer to their homes.See the full content of this document
